     • Photograph (4.5cm × 3.5cm)
     • Signature (in capital letters will not be accepted)
     • Left thumb impression (or alternative as described)
     • Handwritten declaration (in English only, text provided below)
  2. Declaration Text:
     • “I, _______ (Name of the candidate), hereby declare that all the information submitted by me in the application form is correct, true and valid. I will present the supporting documents as and when required.”
  3. Other Requirements:
     • Ensure documents meet specifications in Annexure I.
     • Signature must be in lowercase.
     • Thumb impression must be clear.
     • Prepare for online payment of application fee.
     • Maintain a valid personal email ID for correspondence.

Note: Applications with documents not meeting these requirements will be considered invalid.
